Liability Release:

I hereby release and hold harmless SAFE Net, the agency where I volunteer, and sponsors and supervisors of all activities, from any and all liability for any injury I may suffer (including any injury caused by negligence) in conjunction with the volunteer activity. I also certify that I am in good health and able to participate in the program activities. I certify that I am 18 years of age or older and am competent to contract my name as far as the above is concerned. I have read the foregoing release, authorization and agreement before affixing my initials below and warrant that I fully understand the contents thereof.

Injury and Sickness Liability Release

I agree on behalf of myself, my child, my heirs, successors, and assigns to hold harmless and defend SAFE Net, its board of directors, employees, volunteers, chaperones or representatives associated with the event arising from or in connection with my Volunteer Services Program or in connection with any illness or injury or cost of medical treatment in connection therewith.

No Political Affiliation

SAFE Net does not endorse any political party or candidate; therefore, no political attire may be worn during volunteering to limit confusion among other volunteers and clients receiving our services.
